Problem setting up netgear ta612v

Got my hands on one of these which was suppose to be unlocked but when you go into the web broswer alot of the setting were still filled in with Sipgate. So I D/L one of the Unlock firmwares that Araon had up and updated the unit, it seem to install but not sure if unit is still locked to Sipgate.
Couple of questions for starters plz

1)Where abouts in the web broswer does it tell you what firmware is installed on the ta612v. Because when I went to firmware upgrade it just asks you where you want to install the new firmware from but not what version you have on it

2)Why can I not acces the ta612v's web broswer when I have my pc on a static ip address, but yet when I go into tcp/ip and change it to "obtain one automatically" no probs.

3)My BB setup is this
Dsl fone wire into my Zyxel P-623R-T1 modem / nic cable from my modem to internet connection on ta612v / nic from ethernet connection on ta612v to the pc.
When my setup is like this I cannot surf the net even though I have gone into the ta612v and filled in my BB username/psw but if I take the ta612v out of the loop, no worries. Is there something I need to change in my modem settings?. I noticed in the Lan settings of my modem its setup as a "DHCP server" with options of None or Relay

Any help to get the ball rolling would be much appreciated

4)IF I manage to get the above sorted then I am onto configuring my ta612v so I can make calls. Not sure because it is still locked but when I am trying to add in "Voipcheap's sip details I get an error because I have not filled in a telephone number in a section under line 1. But on voipcheaps webpage there is no mention of telefone no's just user/psw and some sip settings
